Compatibility between the Japanese Odor Stick Identification Test (OSIT-J) and the Japanese Card-Type Odor Identification Test (Open Essence: OE)

چکیده

嗅覚同定検査にスティック型嗅覚検査法(OSIT-J)とオープンエッセンス(OE)がある。これらは検査手順が異なるが,同じ12嗅素で構成されており,近年,広く臨床研究に利用されるようになってきた。今回,我々はこの二つの検査に一致性があるかを検討した。嗅覚正常と自覚している20歳以上40歳未満の男女各10名,全20例の健常ボランティアを対象とし1群は初回にOSIT-J,2週間後にOEを施行し,2群は逆の順で検査を行った。結果:ボランティアの平均年齢32.1歳,男性32.7歳,女性31.4歳で男女間に有意差は認めなかった(P=0.56)。OSIT-Jの平均値は11.0(中央値11.0,95%CI:10.6~11.6),OEは10.7(中央値11.0,95%CI:10.1~11.2)で,両検査平均値,中央値ともに95%CIに含まれた。嗅素別の正答率は12嗅素中,カレーのみ両検査ともに100%であった。同一被検者において,両検査間の同定(正答)・非同定(誤答)嗅素の一致率は,同定(正答)で100%一致した症例は5/20例,非同定(誤答)では1/16例であった。以上の結果より,2検査間でスコアは互換性があると考えられるが,同一被検者で調べると,同定嗅素・非同定嗅素は必ずしも一致しないことから,嗅素に関しては,互換性はないと考えられた。診断としての同定スコアは両者共用で用いることができるが,嗅素間の比較は難しいと思われる。
